How to ensure you get the benefits from our Advanced Directive

  • Keep it updated to reflect the state of your health, the availability of your named health care agent or proxy, and even changes in medical treatments or technology that could affect you in the future.
  • Distribute copies of your advanced directive to all caregivers well in advance. Whenever possible, discuss your wishes as clearly and as specifically as possible with your doctors and other medical providers so that they will have no doubts about your wishes.
  • Likewise, distribute copies of your advanced directive to your loved ones, and make sure they understand and support your intentions.
  • Use a filing service such as Docubank to ensure that no matter where you travel, caregivers will be able to access your advanced directive. Especially if you travel widely, you’ll need to make sure you have ready access to this document should you be far from home when illness or injury strikes
